Each resident has their own form on the computer so all I have to do is click on their name and mark their attendance for the day. Since it doesn't require our initials we can generate it on the computer and then we just print out the sheet at the end of the month and file one in their charts and I keep a copy in my own log book. If you would like I can send you an attachment of the form. Just send me the email address you would like it to go to. I can also walk you through how I do it. Unfortunately I am only selling in Canada at this time but feel free to check out my website at www.activitytracker.ca We have been using this program in my facility for over 2 years and all my staff love it--they find it quick and easy and it provides detailed reports on participation levels for each resident. On each day of the month I write if the resident participated (A)actively or (P)passively. If the resident refused activity I write a ®. On Sundays and Mondays I do a summary paragraph on the computer regarding that resident. Activities and activity calendar must match. doublehelix 0 Report Share Posted June 22, 2008 I have a paper with all residents name on it and room number at the top of the paper i marked off lines to write in the activity and the lines run straight down the paper leaving me little blocks to check off what they have done for a day or write in what we done if it was not a scheduled activity there is also a blank i the top of my paper to fill in the date i have a notebook that i start the month off with putting a calender in a plastic sleeve and putting it in the notebook the i add in my documentations each day after we have the resident council meeting i also put in those minutes behind the sleeve my calender is in my notebook is big enough to hold these for a year and then i file the whole notebook in my cabinet and get a new one to start a new year every year is always a fingertip away if needed to look back at. State says they love my way easy to understand and find everything. I have a sheet with all the res names next to their room number and a code sheet with all activity engagements from independent activities to ss /1:1. All activity staff fill out this sheet every day for activities they did, observed other staff doing, enabled or observed residents doing independently etc. , all volunteers and spiritual care staff do a sheet too. Everyone turns them into the receptionist who inputs the codes into the computer and then prints them out at the end of t he month. Have a great night everyone! I copy my monthly cal and then put my 1:1 page on the back and recopy 2 sided for all the residents. I highlight the cal. for those who particapate and we use a check off for the 1:1. The 1:1 list is 31 days across and it has 1:1 activities going down. If any one wants a copy of my 1:1 list please email me at tmarsteller@wservices.org. After all these years this seams to work best for me. We also have person centered care program so I have a binder for the CNA's to fill out and this is my 1:1 sheet for each resident kept in there break room for them to check off daily. I made my own 1 to 1 form for my staff to use as well as other forms so that I would have better documentation.
