It includes three hospital campuses, a network of primary care and specialty practices, the Cambridge Public Health Department, and the Network Health plan. With this unique model, CHA is able to offer the finest health services, a diverse work environment and a premier training experience to those interested in community-based medicine.
The Elder Service Plan is a comprehensive program designed for frail elders who are nursing home eligible but wish to remain at home.
The program provides general primary care, rehab, and wellness services at an Adult Day Center and at home. It also uses a team approach to coordinate all health, medical and social services needed to maintain the frail elder's independence.
It is currently available in Somerville, Cambridge, Malden, Allston, Arlington, Brighton, Chelsea, Everett, and Medford.
The Elder Service Plan of Cambridge Health Alliance operates under a contract with the Massachusetts Departments of Medical Assistance and the Centers of Medicare and Medicaid Services.
The Activities Coordinator will:
1. Develops and implements a plan of therapeutic activities for each participant to promote optimal health, independence, and self-care.
a. Performs initial, semi-annual and annual assessment of all participants
b. Oversees the assessment of individual abilities, needs, and preference through an activities profile.
c. Identifies problems, establishes realistic goals, and sets up treatment plans utilizing therapeutic activities.
d. Utilizes input from restorative service consultants to improve or maintain functional and cognitive ability through therapeutic activities.
e. Accountable for contributing to and carrying out the recreation care plan for each participant.
f. Plans, organizes, and carries out individual and group activities tailored to the needs and limitations of frail elderly persons following the PIESS protocol.
g. Leads daily exercise group.
h. Demonstrates ability for creative arts activities.
i. Leads regular educational programs.
j. Orients new participants to the program.
Qualifications:
1. B.S. degree preferred in Recreational Therapy or B.S in allied field
2. National Certification by the N.C.C.A.P required.
3. Two or more years experience in an adult social or recreational program.
4. Ability to develop and implement therapeutic programs for both individuals and groups.
5. Flexibility, initiative, creativity, and organizational skills.
6. Physically capable of lifting and handling maximal assist
participants with transfers.
7. Willing to assist toileting and other duties outside the traditional role.
8. One year clinical experience working with the geriatric
population.
9. Ability to drive or transport self to multiple locations.

Love this sample CP -- It is hard to write "I" &/or "me" --when you have been writing Resident will.... ---- The only thing the new CP process is having to be so detailed especially if your facility is large say 130+ residents to attend to -- you are bound to mess up & some residents may slip through the cracks (get over looked)-- Example: Your CP says resident will be infored of food related activites, when new books arrive etc. -- Say I have also CP'ed 20 others for this & still have a entire facility to deal with as far as getting them to & from activites, so MR. B may get lost in the process --The State comes in & discovers that what is CP'ed for MR. B was not done everytime we had a food related activity -- does this make any sense? I know you aren't the person who made the changes but you are the person helping us with understanding the process, just wondering if there is another way to write them without being so detailed???

Hey Diana.. Remember there is always tomarrow.. your hire could have been way worse.. at least it was over quickly compared to some that you dont find out how incapable they are till months later.
A good rule of thumb that always worked for me is... I always hired the Assistant that was incredibly busy in her life .. I always asked the questions that let me know that this person had so much going on that she could barely get any sleep... my dad always told me to always ask a busy person to help.. because they didnt get so busy by saying "NO".. and an person that never says no is the type of person that always made the best Assistant...
