Activity Participation Forms - discussion

Hi.......i am a new activity director at a SNF and i too think the forms we use are very old. i was wondering if any one had a res. participation sheet, room visit sheet, sensory stim sheet, or a monthly calender they would like to share with me for activity ideas. i know it is alot to ask for but i am new and would like to do the best i can to make the residents lives meaningful..... thanx Kim

I am a CTRS by trade, but have recently become an Activity Director at a CCRC, and would like any help I can find on Attendance forms. We only have about 26 Residents on our healthcare unit that we care plan, but it's hard to keep track in any uniform way the attendance of AL and Independent Residents. I need a simplified form if anyone has any suggestions. The only offer I got was a form that I've seen before that shows percentages by counting the amount of activities you have each month, and figuring it the levels of Residents, etc., and it still doesn't make any sense to me--HELP!!!

I was just wondering, When I fist took this job I was an AA. My participation records had 3 months on the same page. Then when I got the AD job the old AD made new participation records that only had 1 month on them. I dont know why she did this and I think it was her own idea. We have not had a consultant for me to talk to. It takes me along time every month to do these and I was just wondering how everyone eles does it 1 month or 3 months. And of course the 3 months would only be good for long term residents. Thanks for any feed back

hi this is kim i am now using my monthly calender as my participation sheet..... i just high-light the activity the res goes to and use different colors to indicate what level of participation ....it is so great you can use the templet on activitydirector.com they charge a small fee for the site but it is so worth it they have helped me out several time already they even have live online help till late a t night im telling you it is great check it out i think it is 9.95 a month but check for yourself.....................and the participation sheet is a breeze now.....Kim

Hey All,

I am pretty new to this myself but this is something that I think I have improved since I got here. I have each long term residents activity participation for the whole year on one sheet. I also made a folder for each resident that includes theiractivity assessment, care plan and activity participation. It is really simple but so organized and it seems to be working great here. If anyone is interested send me an email and I can shoot you over a copy of my resident participation sheets...Janice_Major@beverlycares.com. Or they were really easy to make I just created a chart in microsoft word with 31 columns(days of the month) and 12 rows(months). Then at the end of the chart I made a key such as C=coffee hour, M=movie time...1=active, 2=passive etc. I hope this helps!

Hi, we went through so many forms over the years

Now we use our monthly calendar and put each residents name on a calendar. We have all our programs on the cal and I add a 1-1 spot on each day to record 1-1's. We just highlight each program that they attend. Much easier to eye ball at the end of the month which programs each resident comes to.

Every three months we tally all the programs in the past three months and how many times they attended each program. It sure is an eye opener on what residents slip between the cracks.

Overall this process is less time consuming than alot we tried over the years and you can see at a glance who is getting missed.

Evening everyone.....

I am also interested in easier documentation, I am doing the calander highlighting, but I am trying to incorporate the residents progress notes on it if possible. I would be interested in any facilities forms that you are using that you dont mind sharing. I was put in the Activity Dir. position after our past AD had a stroke and therefor I have not had any mentorship to assist me.

Question for the folks who highlight calendars:

 

Sounds good, but how do you track the independant activities such as crossword puzzles, reading, internet use, watches tv, etc? I would like to get rid of the attendance form if we could find a way to get that tracked.

I have a good idea about keeping those particiaption logged. I have my own separate book/binder. And my facility has a cenus sheet that is generated daily with room numbers and names and i use that daily and indicate the activities of the day in the bottom or top of the cenus sheet and color code each activity and then code ny eack persons name what they attended. State Loves it! It's easy to read and simple to look at to referance back for assemnets and charting your medicare notes and for care plans!

:-D Hi! I am using calendar highlighting and then writing in the independent activities. I then use my progress notes for any other things that need to be mentioned such as active or passive participation, how the resident spends most of day if he/she prefers to stay in room, etc. I am looking for new assessment forms right now because mine are horrible! They are not specific on types of activities or what kind of emotional basis they are in at the time or how their attention span is on interview or anything like that. Kinda hard to write a care plan with half the information i need not on the sheet...If anyone happens to have a good assessment sheet for initial/quarterly/yearly please let me know! I have a paper with all residents name on it and room number at the top of the paper i marked off lines to write in the activity and the lines run straight down the paper leaving me little blocks to check off what they have done for a day or write in what we done if it was not a scheduled activity there is also a blank i the top of my paper to fill in the date i have a notebook that i start the month off with putting a calender in a plastic sleeve and putting it in the notebook the i add in my documentations each day after we have the resident council meeting i also put in those minutes behind the sleeve my calender is in my notebook is big enough to hold these for a year and then i file the whole notebook in my cabinet and get a new one to start a new year every year is always a fingertip away if needed to look back at. State says they love my way easy to understand and find everything.

I use the calendar highlighting system. There are usually blanks at the beginning or end of each calendar. I have a key placed in those to capture the independant activities. For example: 1-TV, 2-music, 3-sensory, 4-visitors etc. I also have that yellow-participated, green-passive, blue-left program, orange-sleeping. I also use the activity calender and highlight. In my progress notes in the individuals chart, I note any type of independant activity that they do. I also keep track in my 1:1 tracking book for those residents on 1:1

I am just new I should say older just restarting again and am going to the calendar idea, on the back of each residents calendar I have made it in to lines for documentation for 1:1's or residents progess for my personal use. What do you all do for documenting 1:1's and who in your opinion would need them. In my new facility its not being done as faithful or as often and am concerned about me not doing it as much cause I am use to a differant way of thinking.

Just wanted to do a quick poll. How many of you have worked in facilities that include the daily participation logs in the residents charts? Did that work well? Were their problems associated with it? Also let me know if you have never worked in a facility that filed these in the resident chart. Thanks, Terrie
