Pennie 26 Report Share Posted June 9, 2008 I am looking for more info from ADs/facilities using an "I" Care Plan. A more "resident involved" care planning procedure that lets the resident formulate his or her own Care Plan.. Residents are getting involved in their own care with the I Care Plan, being asked during the initial assessment about what they prefer, or how they would prefer you to handle their Problems, Goals and Approaches... i.e. residents with pre-existing problems may have a particular method or approach they are familar with and that they know works well for them, so they participate in the care plan with suggestions and preferences. anyone have any input on this,, any forms to share or assessment procedures that help in this area? thanks, Pennie Quote Link to comment Share on other sites More sharing options...

Jen 0 Report Share Posted June 12, 2008 Pennie, Cat Selman briefly mentioned I Care Plans at the Spring In. Activity Director's Association Workshop this year. You probably have met Cat. She's from Ms., is an ACC, & well known speaker. She stated that she has been training nurses how to do them & that they tie into the new Culture Change. There is a facility in San Antonio that is already using them. They are real interesting. They have many examples of care plans for different things such as Mental Abilities, Social Categories, RAP indicated issues, etc and they inclue examples of the "I Statement" Care plans. jenniseaman 0 Report Share Posted July 7, 2008 I care plans. My facility has recently begun using the I Care Plan format. The response from elders and their families has been very favorable. For the activity portions of the care plan I have found it quite simple to express the elders needs in their words. However, the shift from labling "Problems" to 'Interests" has proved difficult for other departments, nursing in particular. Another tool I have begun to develop is the Visual Information Profile (VIP care plan). It is essentially a Powerpoint presentation about each elder and what is important to them in their lives and their preferences in daily care. We use it as a staff training tool. JessRSD 0 Report Share Posted July 11, 2008 Pennie I find the only differenceis the wording...so many of mine are lower functioning that they are unable to be involved in their care plan. I like the I care plan, but find it annoying that 1 problem is not connected to 1 goal to 1 intervention... the way it is now, you have to scroll through 30-some problems, then 30 some goals then 30 some interventions...
