mashonda 0 Report Share Posted June 28, 2007 Please tell me what type of Daily Participation forms are being used and if possible can they be sent for me to see. Thank you I shrink my activity calendar and put each resident's name, room number, and other information at the top of the page and they go into a notebook. When a resident comes to the scheduled activity, i take a hilighter and hilight the activity they attended. If they declined or was out of the facility i include that as well. I have a separate notebook for my resident's are receive in room visits.

AndiLynn 0 Report Share Posted February 5, 2009 Just something else to add. I use the activity calander as well but i have found it very helpful to label what kind of participating they do. I use different color highlighters to label how they did in the activity. It seems more complicated but in the end it has really helped in the annual assessments and the careplan updates. If they are sleeping all the time, if they are really active, if they are just sitting there, if they don't come down at all. It really helps. Here is what i do......Green - actively participated, Pink- observed, Blue- Sleeping, yellow- particpated with extreme assistance (for people who cannot move thier hands, etc) I hope it helps. actnet 8 Report Share Posted August 26, 2020 Click on the word Community on the black bar above and then on Downloads, there are forms in the download area that might help. There are a couple of participation forms.
