sharon53 0 Report Share Posted October 20, 2006 this is what we do at our facility.we print the montly calendar the size of note book paper each resident has their own sheet for that month.then we have four highlighters of different colors. 1-color for refused 1-color for participation 1-color for unavailable 1-color for attended but passive then we record any notes on the back of any activities they do on their own. this works well for us. hope this helps Quote Link to comment Share on other sites More sharing options...

As I stated on a previous post, I'm really not interested in "forms", I just am SO curious if any of you have ever run into problems with STATE SURVEYS concerning attendence/participation documentation. Does anyone know if it is required that cna's need to write what they did with a resident and the response in order to comply with the new OBRA changes? However, we have now added "Guest Books" in the rooms of our residents who we would like to know how many visits that they are actually involved in. We have all the staff signing the residents guest books including Housekeepers and Maintenance. The residents are enjoying making littel guest books and many who don't need them (those that come to all of the activities) are enjoying them as well. I have some forms and ideas but when I took over this position the previous director threw just about everything away so I am flying blind. Guest Mel Report Share Posted January 1, 2007 Our sister facility got a deficincy (I know that that isn't spelled right) for having residents asleep during activities, our new company policy is to try and wake them once if they fall back asleep they get taken out of the activity. Who know's what state will think of that as far as resident rights go. They also got a warning for having to many "passive" residents in a large group activity. Guest bjmegehe Report Share Posted January 13, 2007 One of the monthly magazines for activity directors has software that I have been using for several years. It is wonderful!!!!!!!!!! It saves so much time preparing the monthly calendar and your participation records. It also sort residents by interest groups, etc and prints monthly reports. If you have a computer in your department it doesn't cost much and is well worth the investment even if you have to buy it yourself. Just look in the yellow book. Each resident has their own form on the computer so all I have to do is click on their name and mark their attendance for the day. Since it doesn't require our initials we can generate it on the computer and then we just print out the sheet at the end of the month and file one in their charts and I keep a copy in my own log book. turtle 0 Report Share Posted October 23, 2007 We use the CareTracker system that the CNA's do all of their documenting on. It is set up for activities but requires some redesigning to be the most beneficial. I worked with our IT department to make the program work for us. It makes things easy for my staff and since it is on the network, I can pull the participation logs to review while writing notes and care plans. Another benefit is that nursing and social services can also review participation logs as needed. Our CNA's have also been trained and are assisting us with logging independent activities while they are doing their routine documentation. I pull the logs each quarter and file them but the records can be pulled at a moment's notice. We have been doing this for two years. I highlight in differant colors to signify which they are if they are active. passive etc. For independent activities I also have a number such 1- tv, 2 - visitor, 3-read or write, 4- tv, etc. This works well and also I put in the day if I have done a one to one with them with lines on the back to provide my comments on the one to ones. Have a nice day Thanks, Amy I created a three month participation record that is very simple and covers everything, (No highlighting) additionally, we have the resdients Room Visit schedule on the back (3 months at a time) so that is recorded... and... Attached to every residents Participation Record & Room visit log we have that residents Care plan (a copy of the one that is placed in their chart) If anyone would be interested in any of these they are in Microsoft Word format and are very easy to use and change for every quarter. ALSO...
