Activity Participation Forms - discussion

Hello all! As I'm looking at all of these blogs, all i see is "I'm looking for a new participation sheet, or a new one-to-one sheet for documenting activities and one-to-ones." At my facility, we are blessed to have 'sockets' or palm pilots to document daily activities and one-to-ones. Does anyone else have these, and if so-do you STILL hand document things??? Or not? These are still alot of work, but sure saves writing alot. I just want to make sure I'm doing ALL that I need to be doing. We do chart Quarterly and Annual summary's in the chart, but as for any activity we do, this is documented on those palm pilots or in our computers. If anyone has any info they want to share, I'd love to hear it!

Hi All!

 

I'm new to the site and fairly new to the Activity Director world (mainly because where I worked previously didnt do things the right way). I'm currently at a facility that will be servicing clients from nursing homes and it'll be called a rehab and counseling services center and I'm in charge of activities. And since we're starting from the ground up, I have no clue wat forms I need to have developed for my staff to use! We're in the progress of getting all douments in order. All I know is that I need some type of documentation for the activity aides to document clients participation and have a way for me to keep track of all that info on a monthly basis to cross reference it with the Clinical staff. I NEED HELP! Does anyone have any forms they can lend me to help me out? I've got activity calendars down already but I'm really big on documentation and I want to make sure things are done by the book. I need this ASAP.

Hello all Im looking to move our calendar to monthy instead of weekly, have you all had sucess with using a monthly calendar, or do you just hand outalot due to residents misplacing them.

Hi everyone,

 

I set up a monthly charting form on Microsoft Excel. Each resident has their own form on the computer so all I have to do is click on their name and mark their attendance for the day. Since it doesn't require our initials we can generate it on the computer and then we just print out the sheet at the end of the month and file one in their charts and I keep a copy in my own log book. If you would like I can send you an attachment of the form. Just send me the email address you would like it to go to. I can also walk you through how I do it.
