quarterly assessment

I need to know when a quarterly is really due? From three months from admission or if they go out to hospital and come back or just when the mds coordinator deems them?

I am not sure if you are talking about a quarterly MDS assessment or a quarterly activity assessment. A quarterly review (MDS) assessment is due 92 days after the date of the admission assessment. When a resident is discharged to the hospital the assessment depends on whether the hospitalization or the reason for the hospitalization caused the resident to change significantly. If the resident has changed significantly then a Significant change (MDS) assessment needs to be done. If the resident has not had a significant change then no assessment is needed. This call is usually done by the MDS coordinator. This also depends on whether your facility fully discharges residents when they go to the hospital or if they do a return anticipated (MDS) assessement which the activity department has no part in. If the resident is fully discharged then another admission assessment needs to be done. Also the MDS coordinator can have you do any assessment a few days early to make it fit better in the schedule. Activity assessments are done as follows: a full activity assessement upon admission and at any significant change and a quarterly update when a quarterly (MDS) assessment is done.

All assessments are coordinated with MDS schedule, which is usually every 90 days. Your MDS person should set schedule. A re-admit note is generated within 5-7 days depending on your facility policy. re-admit should not affect the quarterly note date unless as the previous person said there is a sig change. Again, your MDS person(s) should give all team members the dates of the Q notes, Yearlys and Sig changes..

I took over a job where no progress notes or quartly had been done. The building just opened last October and i am trying to get the activity section caught up, hardly any resident has an ininitial assessement, no progress notes, no quartly charting inn the charts except section N on the mds. In one chart the last quartly was done 4-27 on the mds section but no notes, so i thought the 2nd quartly would be 7-27, but the Mds Coordinator has it set for 8-18. I'm so confused.

Sounds like a mess. I feel for you. You might want to ask you MDS coordinator why the 8-18 date when the last one was done on 4-27 as they should be no more than 92 days apart. You say you are trying to get caught up, but you can't do anything that was due before you started your job. I don't know how much time you have for charting, and you need to talk to your administrator or the powers that be at your facility, what I would try to do is full activity assessment when their next MDS date is whether or not it is a full or quarterly MDS, and then another full assessment the next time they have an annual or sig change MDS.

You are correct! Quarterly notes are due when the MDS Coordinator sets the date. If the resident happens to get sent to the hospital during the reference period you may not have to a quarterly note. Nonetheless you will want to check with the MDS Coordinator. The reason why you don't do the quarterly is because the resident could return from the hospital with new needs which may qualify him for different medical coverage (i.e. Medicare, and private insurance). Your residents change in status has a direct impact on the way your facility will be reimbursed for his or her stay. If you ever are confused about a date you should always check with you Care Team and MDS Coordinator. :D
