I have a great one, The Careplanning Cookbook, but am finding it is more towards SNF care plans. Sometimes our pts. are only at our center once a week, sometimes won't come at all for 3 wks. or sometimes they will come on more days than scheduled due to Dr. visits, and so the frequencies on the care plans are what I am looking to manage. MyGait 0 Report Share Posted August 11, 2008 Hello there, Check your local regulation for your specifics, but there is a GREAT thing about Care Plans for Adult Day Services. They really only need to be done when there are changes with the client in regards to meds, condition, behavior, etc... There's no need to have meetings about meetings. I would also recommend looking at the NADSA website for additional resources. Remember that adult day programs are way different than SNF, AL, and other retirement options. Have fun! AmandaS 0 Report Share Posted August 29, 2011 HI Love LVN! There's a care plan manual specifically for the Activites program in Adult Day Care (ADHC&ADC). "Activities Care Plan Manual for Adult Day Services". I think it's the only one out there actually, Adult Day Services is really different from LTC and it's hard to adapt care plans from the "cookbooks" which are devised for a different population. You can order at the site www.totaladhc.com They also have Nursing and Social Work Care Plan Manuals for Adult Day Services too. Good luck! I'm currently the program coordinator for a social day program for early-to-moderate Alzheimer's and will be expanding to add an ADHC. This will be a great resource for my new venture!
