I am not sure how to go about making forms . Do any of you have forms you could share a copy of or tell me where I could find them? Sharon Sharon, I also work in a adult day care and made my own forms.Our clients come two days per week so each page reflects one week. It's four pages per month. The first colum down list the activity. Then we list whether a client was passive or active in that activity and then how much assistance they needed to participate in the activity.( independent, hand over hand, promt and cue or toal assist. It 's a good way to show how a client responds to different activities and if they are having any decline in their ability to participate over time. Hope this helps. I might be able to help. anything would be of great help at this point I seem to be getting pulled in different directions and I have made the calendar and would like the staff to do role playing and also let them know what activity staff are supposed to do in any situation and as for the activities they are to be following the calendar. I have Cna's working on the activity program and they have no clue what the activities are and what to do unless they are held by the hand and shown and I have been out there with them and showed them what to do and they still don't get it. LisaRee 0 Report Share Posted June 26, 2008 One of the first things I tell a potential hire is that I need someone who is able to take initiative to do what needs to be done without having to be told. It takes a special kind of person to do a great job in any activity department and it's been my finding that not everyone is able to do it. Another thing that I tell them is that I need someone with some imagination. If an activity aide has no imagination and is unable to improvise (God knows, things go wrong) then I don't think they would last long in my department especially if I have to hold their hand throughout the day. When I inservice my AA's I go over the above two topics then throw in a few extra things such as adaptive materials for residents with special needs and how simple everyday items can be used to adapt an activity for residents who are unable to use one side of their body or another. Alot of activity duties is just using alot of gool ole common sense...and I've found that alot of folks don't have much of that either! Good luck!
