What Nursing Home Activity Directors Need to Know

An activity director can create an excellent calendar, organize memorable events, and fill a facility with opportunities for residents to connect. But under federal nursing home regulations, one important question remains:

Who is qualified to direct the activity program?

That question is at the heart of CMS F680, the federal survey tag addressing the qualifications of the professional directing activities in a nursing home.

THE QUICK ANSWER

F680 requires a nursing home’s activity program to be directed by a qualified professional. The person must satisfy applicable state licensing or registration requirements and meet at least one recognized federal qualification pathway involving certification eligibility, relevant experience, occupational therapy qualifications, or state-approved training.

What Is F680?

F680 is based on 42 CFR §483.24(c)(2), the federal regulation requiring qualified professional direction of a nursing home’s activities program.

The requirement exists because an activity program is more than a list of scheduled events. It involves understanding individual residents, planning meaningful opportunities, coordinating staff, contributing to care plans, and continually evaluating whether the program meets resident needs.

The professional directing that work must meet the applicable qualifications.

Qualified Leadership

The Four Federal Qualification Pathways

Federal regulations recognize several ways an individual may qualify to direct a nursing home’s activities program. The person must meet applicable state licensing or registration requirements and at least one of the following pathways.

01

Certification Eligibility

Eligible for certification as a therapeutic recreation specialist or activity professional through a recognized accrediting body, under the federal regulation.

02

Relevant Experience

Two years in a social or recreational program within the previous five years, including one full-time year in a therapeutic activities program.

03

Occupational Therapy

Qualification as an occupational therapist or occupational therapy assistant.

04

State-Approved Training

Completion of a training course approved by the state where the professional practices.

IMPORTANT DISTINCTION

These are alternative pathways, not four separate qualifications everyone must earn. State requirements may add conditions, and completion of any particular course should not be assumed to establish qualification without verification.

Does Every Activity Director Need Certification?

Not necessarily under federal F680 alone. The regulation includes certification eligibility as one pathway, but also recognizes qualifying experience, occupational therapy qualifications, and state-approved training.

That does not mean every course or credential satisfies every state’s requirements.

A professional should understand the difference between completing a training program, meeting eligibility criteria for a credential, receiving professional board certification, and qualifying under a state’s rules.

STATE SPOTLIGHT: TEXAS

Texas nursing facility rules address activity director qualifications under 26 TAC §554.702. The rule also requires eight hours of approved continuing education or equivalent units annually.

Texas’s training-pathway language differs from the federal rule. Always verify that the specific program and credentialing pathway satisfy the applicable requirements. The Texas annual eight-hour requirement is not a universal federal requirement.

What Does Qualified Direction Actually Look Like?

CMS guidance identifies responsibility for developing, implementing, supervising, and continually evaluating the activity program. The activity director also oversees activity-related assessment and contributions to individualized care planning.

A professionally directed program follows an ongoing cycle rather than ending when an activity appears on the calendar.

REAL-WORLD SCENARIO

A Beautiful Calendar — But No Qualified Director

Imagine a nursing home offering bingo, crafts, exercise, music, and special events every week. Residents enjoy the activities, and the monthly calendar looks impressive.

But the activity director position has been vacant. Different staff members are managing activities, and the facility cannot identify a qualified professional directing the program.

The concern: Even a busy and enjoyable activity program cannot substitute for F680’s qualified-professional requirement.

What Happens During an F680 Survey Review?

Surveyors may review activity director qualifications when concerns arise about activity programming or the direction of the department.

They may examine whether the individual directing activities satisfies the relevant qualification pathway and whether applicable state licensing or registration requirements have been met.

!
F680 Is an Absolute Requirement

CMS guidance states that noncompliance may be cited even without identified negative resident outcomes.

This means a facility should verify qualifications before assigning someone to direct the department rather than assuming the issue can wait until survey.

F680 Department Readiness Checklist

□ Identify the professional directing the program.
□ Verify the applicable federal qualification pathway.
□ Confirm state-specific requirements.
□ Maintain relevant qualification records.
□ Identify responsibility for activity assessments.
□ Document who oversees activity care planning.
□ Establish staff supervision and delegation.
□ Review resident responses and revise programming.
□ Plan for vacancies and leadership transitions.

Three Misunderstandings That Can Cause Problems

“My experience automatically qualifies me.”

Experience must satisfy specific federal criteria, including timing and the full-time therapeutic activities component.

“Any online course will meet F680.”

Course acceptance must be verified under the applicable regulatory training pathway.

“An assistant can fill in indefinitely.”

Staff may deliver programs, but the facility must still provide qualified professional direction.

Why Qualified Activity Leadership Matters

Activity professionals help residents maintain connections with interests, routines, memories, relationships, and opportunities that give daily life meaning.

Doing that well takes more than scheduling. It requires assessment, thoughtful planning, coordination, supervision, and an ongoing willingness to evaluate what is working for each resident.

F680 establishes a professional qualification requirement. The larger goal is an activity department capable of delivering meaningful, individualized programming.
