chris0531 0 Report Share Posted December 30, 2016 Been in Activities since 1999 overloaded , unorganized , HELP​ . I started a new job at long term care as assist , the AD is very young and learning ...she went on maternity leave and I was left with an unorganized dept having to do the activities , careplans , assessments , mds' , notes , rounds , etc . . The mds / careplan people are constantly on my back becuz NONE of the paperwork was getting done and NO CAREPLANS IN DEPT by the AD so I'm left with being overloaded , I am thinking the admin is going to ask me to direct when she returns from having baby , but very frazzled , any ideas on how I can get organized and get back on track without frustration ? barbaraharrell 0 Report Share Posted January 1, 2017 (edited) I have had to jump ahead of the MDS staff so they don't harrass me. They use to be so helpful when you have so many things to do, especially when it's Christmas time or Nursing Home Week. NOT ANYMORE! I would suggest that as soon as they open the MDS, you get your section of the MDS done first before any other work when possible, either before you leave the day or two before it's due or first thing in the morning. When its new admits get information when you do interview/assessment. Take one day at a time with everything else as long as you are making the residents happy, you steady your pace. Figure out what is the best time of day to do certain things and what's more important. There have been times when I went in to work on weekends when there were no meetings and other distractions to get myself organized. barbaraharrell 0 Report Share Posted January 1, 2017 Care Plans If you are expecting your annual state survey soon, start with the long term residents first. Make sure they are individualized. Start with those residents who have the most issues/impairments - dementia, bedbound/one-on-one, need for special programming, etc. Those who participate in group activities are easier to care plan. I was hired as the assistant and a week prior to actual starting the AD was let go and they asked me to take over. Everything was unorganized and MDS paperwork was behind. I was new to the building and did not know where to begin. I had to take a step back and realize this is a new start and that I have to do things the way I am comfortable doing them and making sure they get done. So, I reorganized the office, communicated better with MDS than the previous AD, came up with new activities, etc. Do things that will help you make the place better. actnet 8 Report Share Posted January 4, 2017 Hi Chris .. they have Activity Consultants that will come in and get your paperwork back on track, you should visit NCCAP.org and look in the section about the MEPAP instructors, they are sorted by state and you may be able to hire them to help your facility if you can find someone local. I was a AD Troubleshooter in my career and I would enter a property, Sit down with several charts at a time and go thru the process of resetting everthing to present. Gather the Careplan team forms if it exists, Doctors Diagnosis, Progress Notes/Assessments, MDS forms. and start a re-evaluation. Start with the assessment and the progress notes and update what exists to bring them current. If you update a existing Progess Note, then put current date and "UPDATE" start your new plan of action from this point on. IF your assesment doenst exist then see the DON for how to proceed, but they better be there.. Once you get the progress notes updated , write a CarePlan and make the careplan team aware that the resident needs to be included. If you get a list of all the Careplan Residents for the next meeting than make sure you do those residents first.. each place has its own system but you get the idea..
