Hi my name is Kathy Gates I am an AD at Horizons Care Center in Eckert Colorado. I have written my activities department mission statement so many times, it is really difficult to pinpoint exactly what you want to say in a brief statement, anyone have any ideas?

Here is an example for you:

  • Plan, organize, and implement life enriching activities based on unique preferences and personalities of residents.
  • Build meaningful relationships with residents and their families as we provide assistance with daily living.
  • Maintain comprehensive assessments and progress notes of the activity needs of each resident and follow HIPPA guidelines.
  • Develop and maintain a good rapport with all service teams involved with care and planning

I just wrote one for the MEPAP1 course I'm taking:
Together we create individual and group spaces for residents to engage physically, spiritually, mentally, socially, altruistically and creatively through purposeful recreational activities that contribute to the holistic well-being of residents.

Enriching the lives of those we serve with compassion, respect, excellence and integrity. Every day is an opportunity for Brookdale associates to deeply connect with people in a profound and personal way.

(I volunteer in the activity department there once a week. I love it!)

Here is one written by me

Life Enrichment Mission Statement:

“Our mission is to plan and implement activities based on residents’ unique interests, background, and skills, to consistently evaluate and improve activities, and to ensure each person’s leisure pursuits, big or small, are attended to.”
