bigchris 1 Report Share Posted April 8, 2009 So many of you have told me that one of the "tough" residents to CarePlan for is the highly active/independent resident. There are several scenarios that we could look at and I invite you to send them to me. But for now, I will give you my CP that I did for a highly active resident that I can't get to come to many activities. Here goes: Problem Statement- " I prefer independent activities to those provided at the facility, I will attend occaisionally so make sure to provide a calendar and remind me of (Food Related) activities. Goal- I will continue to have access to structured activities that I enjoy ( Armchair Travel, Cookouts, Cookie Social) in addition to my independent pursuits ( reading, computers, .....) during the next 90 days. Approach- Please remind me of food related activities when they arise. Ask me about my family and e mails that I receive from them. Please remind me when new books arrive in the library. ETC....... actnet 8 Report Share Posted July 15, 2010 Love this sample CP -- It is hard to write "I" &/or "me" --when you have been writing Resident will.... ---- The only thing the new CP process is having to be so detailed especially if your facility is large say 130+ residents to attend to -- you are bound to mess up & some residents may slip through the cracks (get over looked)-- Example: Your CP says resident will be infored of food related activites, when new books arrive etc. -- Say I have also CP'ed 20 others for this & still have a entire facility to deal with as far as getting them to & from activites, so MR. B may get lost in the process --The State comes in & discovers that what is CP'ed for MR. B was not done everytime we had a food related activity -- does this make any sense? I know you aren't the person who made the changes but you are the person helping us with understanding the process, just wondering if there is another way to write them without being so detailed??? The point is to be more resident based with the care plan and supply the most individual plan for that resident. I am sorry to say that detail is a key and that the traditional one sentence statement is out of the window. Many of the new care plans that I have written are paragraphs. Speaking of the state...the good news is that they have been very complimentary in the past 2 surveys. Speaking of Mr. B- You can offer as much as you like, if you don't think that Mr. B will attend those activities, it may be time to re write the goal statement. Once again, detail is the key. I understand your feeling about having so many residents. I have 150+ with a post acute unit. So I am in the same boat, keeping up is key....
