CharlieJo 0 Report Share Posted January 6, 2006 Are you looking for a script or an outline? Care conferences have a tendency to get out of hand especially if families are not involved. It is important for your team to understand that the information being conveyed at these meetings are for the good of the resident and that if everyone doesn't pay attention and participate it can be harmful. An outline that can be used is that each team member should discuss the resident with the team. This way all can be informed of what is happening in all disciplines. Care conference is NOT for the MDS coordinator to read the MDS to the team, it is to discuss what the triggers are and to inform all. Each team memeber then contributes what they see as a solution.......therefore developing a care plan! It is also imperative to review the CP as a team to make sure that you are in agreement. The actual MDS assessment should not be the focus of your care conference, the care plan should be. As I sit and write this to you, I only wish I could implement this in my facility. I would love to be charged with making care conference what it should be! quick question...... Does anyone do admission care conferences with families as an I-Team? Stacy 0 Author Report Share Posted January 6, 2006 Thanks CharlieJo. That outline will help when I start talking with the whole team. I do have to do a "script". I have an idea of what I want my staff to address, but was just wondering what of say as well. We do admission care conferences for our medicare and subacute residents 1 week after admission. Is that what you are looking to do, or is it something different? We get very good involvement on these admission care conferences. bigchris 1 Report Share Posted January 27, 2006 (edited) Why don't you get puppets or a computer to talk for you?!?!?!? Making a script does not make it professional, it makes it sterile! Care plans are a chance for the family to interact with the staff in a (somewhat) casual setting. Why would you add more stiffness to it?????
