Care Confrences

Recommended Posts

Hi Everyone,

I have been given a task to help make our care confrences more professional. My admin is asking for scripts to be mades for people to follow. I was wondering if anybody uses such a thing, and if they would to share, any type of input would be great.

Thanks

Stacy :hammer:

Are you looking for a script or an outline? Care conferences have a tendency to get out of hand especially if families are not involved. It is important for your team to understand that the information being conveyed at these meetings are for the good of the resident and that if everyone doesn't pay attention and participate it can be harmful.

An outline that can be used is that each team member should discuss the resident with the team. This way all can be informed of what is happening in all disciplines. Care conference is NOT for the MDS coordinator to read the MDS to the team, it is to discuss what the triggers are and to inform all. Each team memeber then contributes what they see as a solution.......therefore developing a care plan! It is also imperative to review the CP as a team to make sure that you are in agreement. The actual MDS assessment should not be the focus of your care conference, the care plan should be.

As I sit and write this to you, I only wish I could implement this in my facility. I would love to be charged with making care conference what it should be!

quick question...... Does anyone do admission care conferences with families as an I-Team? This is not done in my facility and I am trying to get it started.

Thanks CharlieJo. That outline will help when I start talking with the whole team. I do have to do a "script". I have an idea of what I want my staff to address, but was just wondering what of say as well.

We do admission care conferences for our medicare and subacute residents 1 week after admission. Is that what you are looking to do, or is it something different? We get very good involvement on these admission care conferences. It helps the families understand, so when their other conference comes up, they know what to expect.

Thanks for you help.

  • 3 weeks later...

Why don't you get puppets or a computer to talk for you?!?!?!? Making a script does not make it professional, it makes it sterile! Care plans are a chance for the family to interact with the staff in a (somewhat) casual setting. Why would you add more stiffness to it?????

Tell your administrator to contact me, I will talk to him/her....

Thank you for correcting the information that was released. I recently took the test for certification with NAAPCC and it was a very comprehensive and thorough test. It was not a simple easy test to get you certified. You had to know the information in order to complete and pass. I would recommend this avenue to anyone especially if finances are a struggle for MEPAP I & II. I did purchase the book for long-term care seventh edition and without that I would have struggled.

Celeste is awesome!! Anytime i have a question she gets right back with me and she is very informative. At first I didn't know if i was going to like the new course just for the fact my MEPAP course was the own pace and I work Full time plus have 3 kids. But i am loving it! It is very structured and with due dates I feel like i am holding myself more accountable for completed assignments sooner than later. It has actually been a beneficial change for me.. Thanks so much!!!

Good Evening I wanted to let you know that I had completed mepap 1 and had only a couple weeks left of mepap 2 so I had looked at path 2 and have taken the activities test and passed so just sending in the application. I wanted to thank you for giving all your time to us even while Kathy was ill. It didn't go unnoticed, you was a great support and helpful at ideas or even different ways of changing thoughts to positive while feeling defeated. Thank You Stacey Passa AP-BC
