HI Love LVN! There's a care plan manual specifically for the Activites program in Adult Day Care (ADHC&ADC). "Activities Care Plan Manual for Adult Day Services". I think it's the only one out there actually, Adult Day Services is really different from LTC and it's hard to adapt care plans from the "cookbooks" which are devised for a different population.
