mogreen 0 Report Share Posted September 23, 2008 I would like to see what kind of monthly cal, participation records, and 1:1 forms people are using today. I am trying to revise a few things. I would like to see a particiation record with 3 months combinded with 1:1 visits. Is this too much to ask for. If anyone can help me I would be sooooooo greatful. spartans 0 Report Share Posted September 24, 2008 I am at a facility that uses computer for charting. Weekly, I do an assessment on each resident. I use paper charting for my daily log. I fill this out daily and refer back to it when using the computer for charting. I also do a paper copy of my MDS and then enter ot into the computer with a narrative note. If I see someone one on one, I enter it into the computer at the time.

Is there anyway, I could get a copy of the charting you do, both the paper form and/or computer? The system at my alzheimer's unit seems antiquated, but I am new to the field, so I don't know for sure. I am talking them into getting me an office and computer and would like to do my charting there, but don't know how to get started, or what software to tell them to buy? Can you help? Thanks! ILoveMyLVN 0 Report Share Posted September 28, 2008 Hi spartans, I would be interested in a copy of your forms too, if you don't mind. We are also using electronic medical record, and they don't have a set timing for when I enter a progress note. Medical Records said I would write one on "anything significant". Does your weekly charting on the computer pertain to activity goals and anything that other disciplines would need to know about? Do you do any scanning of documents? Thanks!
