Can someone please tell me who completes section F of the MDS? I have heard that Activities might be responsible for this at some point, but I haven't received any confirmation of this yet. Would someone please let me know as soon as possible? Thanks!

Hi, I am in Michigan and I have been doing section F-1 for six years, I think it is in our facility policy. I know other homes the social worker does it.

I beieve that it is a company specific policy . Typically it is social work . However, under the new guidlines for Activities , surveyors want to see that activity has given input in sections AC , B, C, D, & F. Hope this helps . IF you are asked to complete section F and are unsure how to answer the questions ask your MDS coordinator for your MDS 2.0 manual . It will give you step by step instructions

I am so glad that I am not the only one with this problem! I have been doing section F ever since I started in activities 4 years ago. It seems to be social so you really have to work closely with your social director.

Hi Donna,

I have not heard of any changes in who fills each part of the MDS. Activities fills out Section N & Section F is done by the Social Worker/Services. But you need to look at Section F & consider it when planning your CP's.

Hey Me Again

I still think we only do section F as I replied before but I am going to find out 100% for sure for all of us. I'll let you know as soon as I find out!

I too, think it's facility specific. I fill out section C and N. For awhile the SS Director and I decided I would take over filling out section F and now SS is back to doing it.

OK all here is the offical word on who does what on the MDS. As much as I hated to read it!! Read away.

The feds do not care who fills out what section of the MDS as

long as the information is completed by a "qualified professional."

Therefore, facilities can assign whatever sections of the MDS they desire to

whomever the facility thinks is best to complete that section.

Surveyors are looking to see that the AD is included in completing areas

of the MDS that relate in any way to quality of life, that would be all of

those sections that you have mentioned. However, most facilities have

the AD complete section N, with perhaps verbal input to the rest. It is up

to the facility policy and procedure and the MDS/care plan team.

Whatever section is completed by the AD, he/she should be aware of the

rest of the information recorded on the MDS. It is vital that the AD utilize

this information in formulating the input for activities into the

overall plan of care.

Thank you Pam Sanders for all of the help & information Pennie
