Section N Mds

On an initial assessment how do you guys complete Section N of the MDS for change in Extent of Activities Involvement. To me when a resident comes from a home setting this should reflect a major change. However, that does not seem to be what the MDS info indicates. Please give me some feedback on what you do!!!

Hello, I put no change also because by the time I do the first mds they have been in our facility for 5 days. And as far as I know That means would the resident want a change in there daily activities in our facility. I always ask the resident if they feel like they need more activites or are they getting enough. But also Im not really very good at the paper work yet. The don just tells me to read the MDS book in our facility and use it as a refrence. Sorry Im probably not much help.

I agreee...coming from home ought to be a major change...but, I also only put no change...I never thought of it like you said...there is only one instance wher I put major change...that is when we have a significant change...cuz usually that is a precursur to a big change in activities. Hope I helped! :-D

Hi All.. I agree with Christine, the Initial Assessment is to evaluate the resident for the first 7 days.. not including prior living enviroment. The No Change defines the resident as content with current activity routine, and the New Admit fits this definition.. the Slight Change defines the residents content overall, but would prefer some modification to their activity or a new activity and Major Change is for a resident that is bored, restless, isolated, discontent with their activity program.. or could feel trapped by involvement in certain activities and would prefer a change to relieve a stressfull situation. hope this helps..
