Hello, The Elder Service Plan of the Cambridge Health Alliance is looking for an energetic, hard working and Flexible Activity Coordinator to plan and lead activites at our adult day health center, in Central Square, and supportive housing sites within Cambridge. Must be able to travel, own car preferable. Monday - Friday currently. 20 Hours/week. Please review the following job description: Activity Coordinator I Reports to: Adult Day Health Center Manager Main Function: To develop and implement, in conjunction with ADHC staff and the restorative services consultants (P.T., O.T., S.T.), a therapeutic activity program which meets the needs of each participant. Qualifications: 1. B.S. degree preferably in Recreational Therapy or allied field. Activity Certification strongly prefered. 2. Two or more years experience in an adult social or recreational program. 3. Ability to develop and implement therapeutic programs for both individuals and groups. 4. Flexibility, initiative, creativity, and organizational skills. 5. Physically capable of lifting and handling maximal assist participants with transfers. 6. Willing to assist toileting and other duties outside the traditional role. 7. One year clinical experience working with the geriatric population. Performance Responsibility 1. Develops and implements a plan of therapeutic activities for each participant to promote optimal health, independence, and self-care. a. Performs initial, semi-annual and annual assessment of all participants b. Oversees the assessment of individual abilities, needs, and preference through an activities profile. c. Identifies problems, establishes realistic goals, and sets up treatment plans utilizing therapeutic activities. d. Utilizes input from restorative service consultants to improve or maintain functional and cognitive ability through therapeutic activities. e. Accountable for contributing to and carrying out the recreation care plan for each participant. f. Plans, organizes, and carries out individual and group activities tailored to the needs and limitations of frail elderly persons following the PIESS protocol. g. Leads daily exercise group. h. Demonstrates ability for creative arts activities. i. Leads regular educational programs. j. Orients new participants to the program. 2. Responsible for planning and leading of the daily therapeutic activities of the program. a. Plans monthly schedule of activities. b. Assumes a leadership role in directing students and volunteers in activities. c. Works effectively with activity aides and student interns. d. Alerts Activities Manager to supply and equipment needs for the activity program. e. Involves participants in program planning. f. Manages activity rooms and prevents/ defuses participant conflicts. 3. Performs daily duties related at the adult day health center. a. Serves drinks, snacks and meals daily. b. Performs transportation duties as assigned. c. Responsible for set up and clean up of activities and meals. d. Completes other tasks as assigned. e. Stores activity supplies and maintains appropriate par levels in activity closets. f. Routinely purges, redistributes and organizes activity supplies. 4. Responsible for the completion and maintenance of appropriate records and documentation. a. Responsible for contributing to, reading and following participant goal/treatment plans. b. Completes written logs of activity participation. c. Accountable for completing all assigned tasks in a timely manner. d. Completes activity assessments on new enrollees and submits to Activity Manager. 5. Participates actively as a member of multidisciplinary team. a. Works effectively as a member of the IDT through communication and cooperation. b. Participates in ADHC staff meetings. c. Participates in weekly multidisciplinary team meetings when requested. d. Participates in individual/family case conferences as needed. e. Contributes to the creation of care plans for participants and new enrollees. f. Accountable for participating in and carrying out participant care plans. g. Dependable and reliable with a good attitude towards work. h. Arrives on time and prepared to work. i. Maintains participant and employee confidentiality. j. Demonstrates a positive and professional demeanor. k. Adapts to change in a positive, professional manner. l. Appropriately reports participant/family grievances to the team and participants in their resolution. m. Contributes to the Quality Assurance/Performance Improvement Program. n. Demonstrates knowledge and respect for participant’s rights. o. Verbalizes/demonstrates procedures for responding to emergency situation, p. Immediately reports knowledge of abuse, neglect or exploitation to the IDT. 6. Assumes a range of professional responsibilities. a. Attends appropriate workshops and in-service activities to keep abreast in the field. b. Participates in annual evaluations and competencies responding to manager’s recommendations for improvement. c. Attends and completed annual ESP and CHA training requirements. One year supervisory experience in the healthcare field ROLES AND RESPONCIBILITIES 1. To collect and collate new/existing participant activity participation and social information from staff/participants. 2. To complete weekly Bi-Annual assessments for Care Plan. 3. To complete monthly activity/staff schedules and complete other monthly updates. 4. To supervise activity staff and keep notes of areas for improvements. 5. To attend all meetings pertaining to the Activity Department and communicate relevant information to Activity Director. 6. To collect and review activity participation logs and file accordingly. 7. Lead Activities when required 8. Communicate effectively with Activity Director. Rate is determinded based on experience. No benefits are associated with this contracted position.
