I see good replies passed on to you! I will add that you should remember this is YOUR evaluation and it is YOU signing the MDS. When the MDS is reviewed by State workers YOU will be responsible for answering questions reguarding this section of the documentation and must be able to support your evaluation. Don't allow other disciplines to pressure you into rating residents into levels of activity involvement you can't support.
