It's an initial assessment. The OT comes in with a 4 page assessment. They fill out the first 3 pages and the back page is labeled for "activity director". We also have the same 4 page assessment that just has activity director labeled on it. We are just trying to figure out if the OT has to do it first and then have me complete the 4th page or if I am allowed/certified to complete the entire packet.

I recently started a new job at the hospital on the inpatient psych unit. We currently have an OT that does an assessment and then I complete the back page. They were tagged once before for the activity director being the only one to fill out the assessment. I was wondering if anyone knew if we had to have an OT doing assessments in conjunction with ours. What is our scope and practice for activity directors? I have tried looking it up but haven't had any luck. THanks! Tiffany
