Job Specific Duties:

1.1 Interviews and assesses all patients; documents findings in the medical record. Indicator: Initial visit(s) made with patient 48 to 72 hours after admit. Activities Evaluation completed within 7 days of admit. Documentation of assessment findings is completed within 7 days of admission 95%-98% of the time. Completes MDS within 14 days 95%-98% of the time.

1.2 Initiate patient plan of care for each patient. Indicators: An activity plan of care is developed for each patient within seven days of admission 95-98% of the time.

1.3 Implements patient activity plan. Indicators: Implements plan for 100% of patient within 14 days of admission.

1.4 Participates in Interdisciplinary Care Conferences. Indicator: Participates in Interdisciplinary Care Conferences each week 93-95% of the time.

1.5 Documents patient attendance and response to activity program. Indicator: Documents attendance and response daily, monthly and quarterly, as appropriate for individual patients, 95-98% of the time.

1.6 Organizes and communicates a seven day activity program. Indicators: Activity program meets State and Federal requirements 100% of the time. A calendar is developed and posted by the first of each month 99% of the time.

1.7 Coordinates and facilitates Patient Council. Indicators: Patient Council meetings are scheduled monthly at least 11 months of the calendar year. Pt. Council Survey form is used when a physical meeting is not possible. Minutes are generated for Council meetings within 3 days 95-98% of the time Family members, patients report satisfaction with the activities program 90% of the time.

1.8 Develop and maintains a budget for activity programs. Indicator: Activity expense are within budgetary parameters 100% of the time.

1.9 Collaborates with the Director of Volunteer Services to promote volunteer support of the activities program. Indicators: 75% of volunteers assigned to the activities program continued to provide support after 3 months. Feedback from volunteers reflect satisfaction with their orientation and support 95-98% of the time.

 

Minimum Required Experience

2 years or experience in a patient activity program in a long term care facility.

Education / Training

Successful completion of a 36 hour Activities Director certification course REQUIRED.
