Activity Participation Forms - discussion

Smoking policies at our facility is no smoking on premises. They go smoke out in a patato field about 200 yards away from the building. These girls have only 15 minute breaks and still manage to get there and back in that length of time. Thats the power of an addiction. I know I smoked 25 years ago with a habit of 5-10 cigarettes a day, took 5 years to quit. Rita W.

P.S. The AD before me used to do the hi-liter coding thing and got tagged from the state surveyors.

State surveyors (IN) told me this year after I had finally went with the highlighted calendars that they don't like that at all- because they photocopy the charts and all the highlighter does is show up as a black line that covers up the activity-they can't tell what the activity is, let alone what color the highlighter was. But they were nice about it.

Thanks for the reply.. I have been going nuts trying to find anything I have started writing one of my own, but would love to find something to get some ideas off of!!!

Hello I am Joyce, and I am new to the world of charting activity participation. Does anyone have a form that they use on a regular basis that they would be willing to share. I am currently taking over a program that has been doing everything handwritten, and I need something that I can use on my computer.

HI JOYCE - WE HAVE A COMPUTER SYSTEM THAT HAS THE FLOWCHARTS INCORPORATED AND THE ACTIVITY AIDES DOCUMENT ON EACH RESIDENT DAILY. IF YOUR FACILITY DOCUMENTS ONLINE FOR NURSES, ETC, WOULD THERE BE A POSSIBILITY TO BUILD IN A FORM, FLOWCHART THAT ACTIVITIES COULD USE?? KIM HENDRICKSON

my name is missy i just got hired as an activities director and am clueless as to how to write progress notes.the ones already in the chart are not long enough and need to be more detailed but i have not been to classes and have no way of knowing how to write progress notes.really wish i had actuall examples to look at.

Missy-the best advice I can offer is to be sure the notes are individualized. I try to write mine so that a complete stranger could get an accurate picture of what that resident likes to do. Things I include in my notes are group activity participation, response to 1:1 visits, independent activities, does the resident need reminders of scheduled programs or assistance with particular activities, hearing loss, vision problems and a review of the activity care plan goals. I try to be specific as much as possible, including favorite television programs, times of day they are more active, etc. Hope this helps! Good luck!

I am fairly new AD at an indipendent community. Participation is minimal. My hours are only 30 a week. How do you find time to do a survey and attendance chart!?? Any suggestions on how to do it all!?

I don't think you have to worry about charting for independent residents. We are a continuing care facility, and my counterpart in the independent side (called the "Resident Services Director") does not chart anything for them. I believe she keeps track of participation as in, "14 people went on the museum outing," "35 people came to Casino night," etc.

 

And as for the poster who said they were stumped as to how to write progress notes, I'll give you an example. Here's one, word for word, that I wrote on one of my Skilled residents who is mobile and participates in AL activities.

Res. is very active in activities. Attends AL walking club and exercise almost daily, and also likes bingo, music, games, and social events. Very forgetful and easily flustered, but very pleasant and cooperative. Goes out with wife approx 5x a week, and they also attend AL outings 3-4x a month. Res. may become agitated and restless when he is worried about something upcoming; staff reassures him that he is not late, and directs him to his calendar and clock to see how much longer he has. This may need to be repeated often.

Res. is content with current level of activities.

I think anyone reading that would get a good snapshot of what he's like, and how to approach him. Does that help? As for progress notes, I write them on each monthly attendance log, like a monthly summary and review, then put it in the chart; unless there is a major change or issue, and I write that in the chart immediately. For example, here's what I wrote when we had a problem with a resident on outings.

Today res. became very agitated and disruptive on scenic drive outing. Began cursing at other residents and chanting constantly. The disruptive behavior is normal for her, and has been seen at other activities and outings, but this was extremely hard on the other residents. Res. will not be taken on group outings for the sake of the other residents. Res. has two private sitters that take her out for coffee, lunch, and drives about 3-4x a week, so res. is still able to get out of the building frequently. Will possibly attempt another group outing in the future if resident's behavior improves at other activities.

(Side note: It hasn't. She is the thorn in my side, constantly chanting God help us and snapping at other residents during activities. But her sitters bring her to everything, and it's a battle to get her removed without compromising her dignity. What a struggle.)

Hi, we have one that hold 12 months; 6 months on front and 6 months on the back as well as putting on independent activities on the bottom. Which is really nice, because you dont have to have mulitple sheets for one residents. THis form was created a number of years ago, and it's really user friendly, we use numbers for activities, and have a master sheet of what numbers go to each activity. Hi everyone,

 

I set up a monthly charting form on Microsoft Excel. Each resident has their own form on the computer so all I have to do is click on their name and mark their attendance for the day. Since it doesn't require our initials we can generate it on the computer and then we just print out the sheet at the end of the month and file one in their charts and I keep a copy in my own log book. If you would like I can send you an attachment of the form. Just send me the email address you would like it to go to. I can also walk you through how I do it.

I am currently trying to create a Participation Form as well as a Activity Assessment. I have accepted a position as the Director of Activities at a brand new Adult Day Center. I have always worked in LTC and seem to be making the Assessment too similar to LTC. If anyone would be willingly to share the forms they use in Adult Day Centers, it would be greatly appreciated.

Hello Everyone!! I am new to this site and am very excited about being here.

Anyhoo I digress. This is actually in regards to logging participation.

I have a form that I HATE yes I used the H word and put it in caps! Unfortunatley I have to use this particular form. It is grid form and makes my eyes cross.

I was given the name of (and I forgot it) A software that is available to use on your pc. I believe it was around $130.00 ish in cost. Does anyone know of any such program?

I can't use it in lue of my current form but would like to find it to use as an assist.

Any help would be greatly appreciated.
