This form should be use to track and document your residents participation in your activity program. The form uses a coding system to note the participation level & the residents reactions, all vital information when designing a plan of care for your residents. Be sure and keep your records up-to-date, keep 3 months of records close at hand for State Surveyors. They usually ask for 4-5 resident files to base your program on, this is a random choosing unless there were complaints for a certain resident.Free- 2 comments

I've replaced an Activity Director who retired. At this facility they have been documenting lunch (only in the large restaurant style dining room where residents are independent) as a social activity in the activity participation record. Is this a practice that happens at other facilities?

Hello all I'm in need of everyone's professional collective wisdom on this one... Presently, my staff are utilizing a very detailed excel spreadsheet that lists all of the activity programs our residents may possibly engage in during any given day within our home each month--there are even additional slots for special impromptu events. This includes spiritual care, group programming [large and small], 1:1, independent/self-directed, staff led, etc. We utilize single letter codes to indicate the residents’ degree of involvement and have additional spaces for further documentation on th
