    Help!

    As a Rec Director, I wish we had a good computer and internet for my residents-I have a put-together one with printer, but right now, we have people who are not interested. It's hard enough to get them to play the Wii instead of watching one or two do it. Today a man told me to do it instead of him. Anyway, I have said that when I am a resident, I want a laptop (hope I have one by then!), a printer and a good wi-fi connection as well as adequate electrical outlet (can't have an extension cord!). I wouldn't expect a facility to provide me with the hardware/software, but I'd need the hookups. And I seriously doubt I'd want to be out playing bingo when I can get comfy in my bed with a big bed pillow, my tea and a laptop with internet! just careplan me. Perhaps you could have an internet scavenger hunt-first one to find a certain number of particular sites (using google or other search engine) would have to email you and win a soda! Or they could work with another resident who has family far away, and send that family member an email for them. If they have powerpoint, have them put together a slideshow , especially if you have a theme, i.e., racecars, horses, trains, penny candies, sodas, old advertising, etc.

I do hate to toot my own horn, and I already am tired from planning too many parties in Dec and gearing up for Valentine King and Queen, which leads into Easter/St. Patricks, then right into Volunteer and the NNHweek! (I'm not a party person, I'm more into individualized recreation therapy. Last week survey was in and the whole year was pretty shaken up-yes, I got a tag on small groups and 1:1s, which, ironically, is what my preferred calling is, but hard to be everything to everyone. Spending too much time being a gopher and not enough doing these neat programming ideas I have...I hung the poster and gave my assistant a shirt, plus took her out-but only the therapy dept. noticed it. I make sure my SSD gets noticed during her week as she also tends to go unrecognized-and activities is expected to have the "residents" (read one resident and I re-do it to be nice and finish the rest) for Nursing or cna week. I agree with the NNHWeek-that's the one time someone actually looks our way, and I think that's just because the administration is expected to show some appreciation or involvement, so it might trickle down some. Here's hoping for a better year, good surveys, and more involvement from those who don't realize how vital you are to your residents and the staff! Happy 2009!
  3. Jess, I think we work for the same company. I do love the I care plans, but you don't have to use only specific interventions/goals. For example, I am new to the facility; goal being will adjust to my surroundings and make friends and find ways to spend my free time; you could use several of the interventions, without just staying within one section. i.e., encourage to sit with others in common area; invite to DR for meals; encourage me to sit in activities and join at my own pace, as well as encourage me to respond verbally to others. You can flip around; they are kinda grouped together to be easier to choose from, but not limited to that section. Also, Jess, you can always start with them and then tweak them to your person; that is definitely person-centered care!
  4. In October I put up a current Resident list and have staff to sign up to adopt. They bring back a wrapped (usually gift bags are best) gift, sometimes they put their name on them, most don't. The Residents don't usually pay attention unless they're really with it. Then, the local Pamida store has an angel tree and we give them about 20 names that are left. They usually buy gifts for those that aren't taken, very nice hometown store. I get them back and sort them out a day or two before the party, like the office isn't a disaster already! But, the night of the party, most residents come out and enjoy the evening, Santa comes and several families use this night as their Christmas exchange with them. For Christmas morning, I purchase a $1 gift for each and give them to dietary; they put one on every tray so they can wake up with a gift then.

State surveyors (IN) told me this year after I had finally went with the highlighted calendars that they don't like that at all- because they photocopy the charts and all the highlighter does is show up as a black line that covers up the activity-they can't tell what the activity is, let alone what color the highlighter was. But they were nice about it.
  6. I had a similar program a couple years ago. I ordered b&w pics from one of the suppliers-GaryGrimm or Nasco, don't remember. I called the program StoryPics (not sure if Timeslips would be a copyright problem). I would try to blow up the photo but of course it doesn't do too well, but also used the original 4x6. I would tell them to each take a person in the photo, and invent them. Give them a name, I would ask questions, i.e., single, kids, profession, live in country or city, etc. and they would tell me what they are doing there, i.e., on a train-where are they going, who are they meeting, etc. This did pretty well except for a couple of things-one lady who leads everything (we all have one of those) would take over, and some who parrot would only repeat what she said. Another problem was: I would write the story down and print it in the newsletter. It turns out, one lady noticed a man in the group, and gave a person in the photo this man's name. When his wife, who was so upset over him having to be placed in the home that she didn't visit often, it hurt so much-read it, a daughter asked me about the circumstances, as her mother was fearing that another lady and her husband were interested! Oh, my, at least the lady did come and we discussed it and she eventually was able to voice her thoughts about missing him and she actually started visiting more often, and enjoying the visits, and the daughters were able to tell me, after he had passed, that it turned out in the long run the best thing, as mom spent more time and had less to regret than if she had continued to stay away. So, that was an odd side effect that I never expected, jealousy, but it ended well! Whew! We haven't done these in a long time, the stories all started to sound the same, led that "the leader". Maybe it's time to bring them back again, with a new crew-maybe as a pre-meal!
  7. The first 4 years I was here, the mail was brought in by A-receptionist, B-business office manager or C-Administrator, in case the others weren't there, and sorted by that person. Only Residents' mail and my own were in my box. When our last BOM was hired last year, that changed. We went without one for a month, and when the new one was hired, she evidently wasn't told about whose responsibility it had been. I continued to get it. She recently lost her receptionist-had complained about her messing things up-so they sent her to me to replace my assistant who quit. This lady is 74 and does very well with 1:1s and small groups, so I'm happy. But I still get the mail. The BOM even told me once about how I should separate the mail into boxes. Of course, she has so much to do that she can't do it, so why bother arguing?
  8. Hi! We did this a couple years ago with a lady who was 110. First, letter from the President-do this in advance, forget if it needed a whole month or what. Because of this lady's age, I made contact with a man who keeps an updated list of national and world supercentenarians (google that and you'll probably find him). We invited mayor of town, all her family, her church (she was from the county next door), and congressmen. Mayor came and spoke, handed out certificate as he proclaimed it her day, nice big cake, flowers, corsage, made sure her hair had been done and she was in nice suit. Get your marketing director involved here. Everyone had a nice time, she opened presents and was very excited. Good luck!
  9. Indiana 5 years in field, Associate Degree Recreational Therapy. 2-1/2 yrs. as assistant, the rest as Director. Started as assistant at 7.25; director at 11.25; now at 11.75.
  10. Oh, Diana, I love that! My Activity Room is the back portion of the DR, and I only recently got back some 4' high portable walls. However, staff usually go through to the back door to smoke on their break. Most of them are considerate and make little disruption, will apologize if they come inside after we have started. But I have one in particular who actually continues to take her trash out (shortest route) this way during activities, take her smoke break, stop and talk to residents each time, and have already told DON, DCE and ADON. No results. One other CNA told me that person said no way was she going the long way out for trash. And with all out turnover, I can't risk being accused of being rude to her, regardless of my personal opinion of her work ethics. However, I do like this idea of "pay a quarter toward bingo if you come through during an activity". That will be fair, I think. Thanks!
  11. I have an Associate Degree in Recreational Management-Therapeutic Option. I never took the 90 hour course for AD in In., so am not certified, but qualified by the state. We now have a Recreational Therapy Consultant, whom I love, and although we know about the F248 changes occurring before the other disciplines do (and therefore are not taken seriously, unless we have a fall/elopement risk/behavior problem and they call me to drop everything to do a 1:1).. Right now, I am called to do what is considered the most important-to be a department manager. That means to help pick up where others leave off because they're too busy-State's been in here all year and staff is leaving right and left. I repeatedly am given a new schedule of another important list of things to do (along with most of the other managers, though we are currently missing 3 of them), such as alarm checks and door checks, water temp checks, etc. Don't think anyone notices whether activities are happening, unless they have a behavior to push into the room. Money isn't my worse problem-it's the hours I have in my budget to work-so, I work about 1/3 of my time off the clock/at home. When my really terrific assistant left after 2 years, and I was trying to hire her replacement, my DO stated that I should only be making $10 an hour. When I was hired at this, I was given 11.25, and the one across town who started with that company started at $13; and she has 3 assistants, less beds. Am praying about whether my life as an Activity Director is meant to change somehow-maybe flipping burgers at McDonald's? Then I could volunteer here and actually accomplish what I have been trained and have the heart for!
  12. We have used Light and Lively for the 5 years I've been here, plus I don't know how long before that. I once had gotten an exercise video from the Arthritis Foundation that I thought was going to be perfect, and tried it one day-a lady said she liked Light and Lively better because she knew it. So much for variety!
  13. IND: Just residents vote for council officers. Being an election year, you could make a pretty big deal out of this. Be aware of that "One" person every home has, will want to run, and ours turned out to be a vice president, meaning she gets to feel important without being the top gun. During last election, I handed out copies of a paper for people who wanted to run for office, then those turned in, I typed up ballots to be turned in to me. A few who are alert and oriented but don't like to attend council, did vote, but it's the regulars who come and half of them are the officers, so there wasn't a big competition, but the "one" felt like a winner, and that would have been the only "sore loser" at our place. All's well that ends well. Our president is losing ability, but can still function by asking for old, new minutes, asking for pledge of allegiance, closing meeting, etc. We use a back scratcher as her gavel and she loves that! Our state surveyors always ask me to go to council members and invite them to a set meeting with them. They might ask staff who the council pres is; they usually ask for 3 months council notes. As far as staff in meetings: Since none of our who are interested in council could accurately record details, I do take the notes, the secretary signs them-has to be reminded how to spell last name, but penmanship is nice! Also, we have in the council notes that the Rec Dir. and/or Asst. is always invited to each regular monthly meeting to keep the minutes; one of the questions each month is whether they wish to have any other dept heads at the next meeting; doesn't usually result in anyone being asked, but that way, there is an open line of communication documented that is in place. Sometimes they might ask for dietician when she comes. Our company's forms ask that each resident sign in; it is in the minutes that I write their names down so they will preserve dignity of those who can't write, write badly/slowly, as only 2-3 could write their names well, and the patience level of the others might cause people to show exasperation. It is my understanding that an issue with 2 or more Residents agreeing is a council issue; if only one Resident has that particular issue, it is given to SSD as a grievance and handled separately. This was just gone over with our Rec Consultant and SSD.
  14. Get some of the Nature print papers (Nasco, but I think S&S may have them, don't remember). Gather leaves, flowers, twigs, and let them arrange them, you put them in the sun for 3 min and bring back their art. Inexpensive plastic frames would be handy to have if they are really pleased with them. They can spend all the time they want arranging them as long as they aren't out in the sun, and again, the process of arranging is a great way to succeed. We have a lady who has a terrific pencil drawing in her room she had done. She will tell you she had done it, too. She enjoys colored pencils and coloring books, I use the floral themes. I had also bought her a kit for Christmas with charcoals and good paper, like she was used to. She enjoys both, no problems with State once they saw the careplan and her history, even though it had to be pointed out to them.
  15. Thankx! Will be adding FlyBall to August! We have started Wacky Ball. Original idea sent in was to use pool noodles and a punching bag balloon. I used a beach ball and cut the noodles in half, just seat in a circle and they whack that ball around! Sounds too simple to be fun, but it can really get their arms moving, makes a lot of noise but no one gets hurt, and best of all, the only ball game I don't have to chase the ball, since they can all reach it. Have also used it while seated at tables pre-meal, and just direct the ball around the room, using a noodle myself.
  16. I went out of the box this year. I had printed off the AHC "Love is Ageless" items, then noticed that our company was coming up with a "Be Proud" line for general use. We had had a name change, an admiinistrator change, and it seemed appropriate to try to psyche up the building. So I printed up my own version of "Bee-attitudes", ordered some stuffed Buzzby Bees on clearance for a dollar each, hung them from the ceiling, drew bees and printed one of my Bee-attitudes on it (Bee-autiful, Bee-dazzling, Bee-lieve, Bee-loved, Bee-gin, and a few more); I put together a ceramic mug, a small jar of honey and one of the bees, to give away for the spirit award of the day for staff, criteria being dress, participation, assisting residents. This was separately done with a 40s, 50s, 60s, 70s and 80s day. Dress, music, movies of that era. Scoured the Net and made a poster for each day using pictures, phrases, music groups, movie posters, etc. and placed a new one outside the dining room (activity room) entrance daily. Facials and nails with ladies' tea party; preschool carnival with name that tune and hula hoops; Bunco and the home ext. lady came with honey recipes and discussion (the local beekeeper didn't return calls, but that was a plan); Bee-ngo! A family night party, Bowling, Craft and bake sale. Not sure my spelling bee will work out, as my best speller is in the hospital, and without her getting others pumped up, I doubt anyone would do it, but we'll try it.) Am ready for a looong nap now.
  17. Tuesday is the hairdresser's day to come, so our attendance is scattered, especially in the morning. I tried to set up a Men's club during that time, but our men tend to sleep in or just say No. (they only come for bingo and food, and if the ladies found out I give them that, it won't be a Men's group!). So, we call Tuesdays "Beauty Tuesday." That is also the day the church ladies come and make and serve popcorn and pass it out (though most choose store bought fluff). Then, in the morning, as people are getting out and about, we have "Fancy Fingers"-filing and painting nails, or giving hand butter if they don't want polish. We aren't allowed to clip; nurses must do that d/t potential for making ouchies! And that's fine with me. This week, during National Nursing HOme Week, I had a theme of "Bee Proud", using bees, honey, Bee-attitudes, etc. So we did "Bee-autify" which was floating a package of facial cloths in warm water, then I brought them in, used a separate cloth to wash each face, went back and let them choose a face cream, applied it, then made the rounds with the nail polish, then the hand butter. This went from 10:00 til 11:20, and we were playing soft 40s music (also had a daily theme of the decades), they enjoyed it regardless of cognitive level. Just doing makeup doesn't work with me, as most days, I am it, and the better part of the morning I am in meetings. Lot of take home work, can't do it all. Now, if the staff were actually bringing people to activities that were marked on their room calendars, or if they were out of bed for those they wanted to come to more often than not, I would possibly be able to help out in that area, too. As it is, I do a lot of things that the DON thinks the staff does, but just doesn't happen in the real world. The residents are what's important, and if I'm meeting one need and not another, at least I'm giving them something they do need (could be why I had a tag this year for the first time?) You can find the aides that are very helpful, and when someone really looks great, go all out to tell their aide what a wonderful job they had done, maybe even that the resident bragged on them about it (even if you have to ask the resident if they are happy with their look that day, in order to get that compliment for that aide). A little honey does go a long way, if they do something that is supposed to be routine but they think is extra, give them that extra recognition and it might well turn back into routine work and that is good for the resident.
  18. We have an adopt-a-Resident program. First, in October, we put the list out for Staff, after they adopt, our local Pamida store asks for a list as their "angel tree" is specifically for area nursing home residents. Of course, in those three months, the Residents will change, so the night before the Christmas party, I sort through and even out if someone gets very little and others have everything on the lists (instead of "choose from", as many times happens). Last year, we had 3 admissions the week of the party, so we did do some quick shopping. Also, I find something around $1 that is given out on their breakfast trays on Christmas morning-I have found great stuff at Oriental Trading and SS Worldwide; one year there were nylon bags that folded into a matching zippered coin purse with a sports ball printed on it; they loved those; Last year, I found some very pretty glass suncatchers; this year, I have found dry-erase boards with an animal border. The facility used to have calendars printed, but I find that families tend to like to buy them ones with pictures they like; as far as the dates, they have the activity calendar and it is printed larger. But, they didn't have the adopting program then, either.

  20. I just wanted to let you all know about the new Reminisce 3-DVD set, the Reiman publication Reminisce and Reminisce Extra (as well as Country, Country Woman) magazines. I ordered it and haven't even shown but 2 segments-gave our group a good hour and a half of terrific discussion!! Usually, during our Reminisce activity, I will bring in my magazine from home, read a segment, ask if others experienced that, but usually they begin before I ask anyway! This time, we used the first DVD and during the segment, I would pause the dvd and we would discuss. There are 3 discs; the first one is shorter than the other two. I will admit, they do show some repetition of photos, but when done over several weeks/months time, it won't be noticed much. This first set is the 1940s, One has The Decade, the next is Our Photos, the third is Our Stories. They interview people on various subjects, such as love, on the homefront, cars, jobs, etc. They include still photos people have sent in, as well as some old home movie clips. Some photos are shown with background music of the times, some with the people talking. Newsreel clips, film stars, commercials, new products. My husband and I watched this before I took it in and it is really worth the $30! This is certainly not anything surveyors would have a problem with as all the discussion would probably occur without a moderator, but since a remote is needed to go to the next segments, it might not be one to use when you are away, unless a staff member would actually check often to see if it needs to be set again. I actually make my calendars and newsletters at home, using my Printmaster program; (Word tends to get quirky about changing the size of the squares and messing up the page) simply because I can think better there, and don't have Internet access at work. I then put in on a thumb drive and bring that in to our activity computer, which is an old computer from when our company switched to new ones, but is no longer on the system, no security problems that way. I had an older version of my Printmaster loaded on it so I can continue to add as needed. I can print it on 8x11, on a cheap printer hooked up, then take it to our copier and enlarge to 11x17 for the individual room calendars on their wall bulletin board. That is highlighted for each person's preferences for staff and for state's requirement; however, those who read are given a second copy, same size, to keep with them-too hard to read on the wall. The 8x11 size is included in the monthly newsletter. Our 2 hall calendars are actually amazingly nice - it's a grid that holds an 8x11 page for each day-those I just print out by page, make 2 copies and tack them up.

    Bowling

    We actually have the plastic bowling set with a 2.5 lb. bowling ball (rubber I think). Our bowling is every Friday morning, we bowl 3 frames. We have 2-3 people who can stand; the rest sit; however, we had a bowling ramp donated by a resident who had trouble getting a good swing and I was mentioning to our volunteer one day they make a ramp- well, this guy overheard and sent his wife to me to find out how much it cost; now we doubled our participation (bless his heart, he just passed on this week). Now, the problem began: I tried to make 2 separate times, ramp and without, to keep score equally; well, you know, not everyone comes when they should, some come early, some late, even when I called it "orange team" and "green team" bowling. So, I had to let them come (okay, reality is, I had to go get those that were up and ready and HOPE someone might look at their calendar marked that they like bowling and bring them down); then start. I give out stickers I print up, with Orange Team and Green Team bowling, 1st, 2nd and 3rd places. Those with a ramp get the orange, those without get green, and I write their name, date and score on them, as well as if they got a strike, or first time bowling, etc. Some of our people choose to throw it like a basketball; they can still use their arms but can't do the backswing; on e day, a lady who uses a ramp was just really weak and not very alert, and we actually joked we would have to use her knee if she didn't wake up-her knee actually came up and pushed it off the ramp, and she enjoys being told about bowling with her knee! I try to get people not to use the ramp if they're on the borderline. If they don't use it, I give lots of praise and may even write - on their GREEN sticker-"didn't use ramp today". These are the big 2x3" labels I print up, and stuck on the w/c arms, they can be popular all day-or week, until the next game. I just have trouble scoring the strikes/spares when they gang up in a row- if they get a strike on the 3rd frame, I give it a 20; a spare on the 3rd gets a 15; but it's the ones inbetween that I probably don't score the same from one game to another. But if you can get a ramp, it sure will include almost as many levels as a music event can, with good participation! Oh, I have large stickers permanently (more or less) taped on the floor for placement; then, a pink sticker for where the w/c should be; and a yellow sticker to mark standers as well as those in gerichairs or w/c with the ramp. Have fun!
  23. Question: Since the idea is to make it a specific "Fathers' Day, focus on the men", event, is this for men (with or without their families,or for all? I'm asking because, with the high female to male ratio, leading to men reluctant to enjoy group activities (don't want to be with a bunch of women), if it's supposed to the about Fathers, or men, then it shouldnt' be for the women; however, the usuale core group of 4-6 women will always want to check it out, and how soon they forget the nice Mother's Day tea party with a flutist and pianist. When state wants to know what we do specific for men, how could we include the women and have it count by their standards? Or, conversely, since "everyone is welcome to attend all group activities, or it's not a group activity, how could they not be included, by state standards, which, of course will be a 3:1 female to male ratio attending the father's day event? Just wondering how you all handle these things. Women come to anything that's happening; men have to be interested to come.

Weekly Meetings:Monday - Friday 1/3 to 1 hr standup meeting Tues Careplans, weight, grand rounds Thurs Marketing, Grand rounds Monthly meetings: QA - quarterly have docs invited with a meal; other months just dept mgrs.; Resident Council; Meals on Wheels Every Mon-Fri, I transport Residents from 10am activity to their DR table-my "activity room" is the DR - and then get those from the lobby , put on shirt protectors, pass trays - then I can go get mail, pass it, and do some charting while I answer phone - office person at lunch and secretary comes in later (hey, at least we have one again now!). Try to get a half hour lunch in so I can get back in time for afternoon large group - lots of stopping to assist residents in ways that they don't want to "bother" anyone else cause they're busy Since nobody really claims our courtyard, during nice weather, it is assumed to be an activity, though we only go out there during the few nice days (not too cold, not too hot or humid) in spring and fall; my residents love to watch me work and ask me to go get them something or they have to use bathroom and believe me, there is no calling for someone to come get them, I simply have to leave to do it. One part time staff; she does the Sat. for 1-1/2 hour and one night wk-volunteer does the other night and Sun p.m. or else it's a movie if no volunteers accept that week. Phone calls, scheduling parties, newsletters, calendars, printing, overseeing resident volunteers that put them together, lots of DR chores tend to get thrown my way. All the above is just what I have to do so I get to do what I want to do, which is to love my friends who live here.

    What to wear

    Recently our therapy staff was requested to wear scrubs, but not the Rec dept. We wear slacks and nice tops, casual enough to move in but nice enough that visitors know we work there. If we're expecting visitors or have meetings, a suit can be worn, but my residents know that just because I may start off with a sweater, long-sleeve shirt over a top, or jacket, doesn't mean it will stay on all day! On bowling day, it doesn't last through the first frame! Shiny or fuzzy tops generate a lot of interest and stroking; I wore a skirt for Christmas eve and was complimented by the men as well as ladies; Friday is jean day, nice t-shirts are acceptable that day only. New Years eve party is luau, so I'll have cold toes with sandals on in the evening, but I won't be wearing them all day!
