We are opening a new Adult Day Center at the end of this month. I was wondering if anyone would be willing to share the forms that you use when you admit a guest into your program? I am bit overwhelmed with all that needs to be done and could use a little help! Any other forms that you have found useful throughout the day would be great too.

Did you have any response to this? I have accepted a Director of Activities for a brand new Adult Day Program. We hope to officially open first week in April. The Director and myself are both creating our forms and it is overwhelming. How is your program going? How many clients do you all have since opening? We are located in St. Louis, Missouri. Would love to hear back from and hear about your program. Also if you would be willingly to share your forms, would love to see what you all use.

I work in an ALF, so I'm sure the forms would be different. I hope all is going well with your new day care center. I can offer you free sample question and answer pages to use in your center.
