Hi Jersey,
Will you send me a copy of your Excel Part sheet. If you agree I will put it up on the site under the download area for all to look at etc.
If anyone else has any type of forms that you have made up and find useful please email them to me as well. I will put them up to so that we can share these. If you know how to put them up please do this. It helps so many of us out. Hello there, I just was reading the attendance documentation procedures some of you use out there and I wanted to remind you of the new MDS3.O that is coming down the road and from your attendance sheets, you will need to chart how much assistance the residents need while in your activities. ie. Independent, minimal, max.
Just a little heads up!

Creative Forecasting(monthly activity magazine) has an Actvity Planning Simplified Program. You can call for a trial version(CD Rom) at no charge to see if it works for you. The number is 1-800-373-0115. It has a 12 month calendar, daily activity schedules, group and individual participation reports, birthday records, daily trivia and more.

I have created my own Activity Participation Log on Microsoft word that I find to be very helpful, Also I have developed my own Room Visit/One-on-One charting form that is very helpful. If interested in a copy please e mail me at msarasa@thevillagehemet.com and I will respond immediatly with a copy.
Both hold three months of information.

Speaking of the new MDS 3.0, I just got back from a conference where I attended a class pertaining to activities and they mentioned the new regs coming down the road concerning Alzheimers units and acts. The new thing is to document all the "moments" you have with the residents instead of having just big group acts. My question is - how do you document all of those special "moments" you have with your res. throughout the day? Our dementia unit is a 16 bed unit with various levels of dementia. I have usually 7 - 12 res. in a group act. morning and afternoon (9:00 - 11:00) and (1:00 - 3:00). The leader of the class suggested doing away with group acts. and concentrate on individual needs of each res. and having lots of "special moments" with them all. HELP! :-o

I am struggling with my activity attendance form right now. I am currently having to hand write all resident's names and then hand write their participation levels. I am the only activity person at my facility of 70 residents. I have three separate units to keep up activities and charting on. I was wondering if anyone had anything on excel that might help speed this process up a little. Handwriting attendance records for 8 or more activities a day is so time consuming. Is there anyone out there who is going through what I am or may have a better idea? PLEASE! Thanks,Kimmyh

I totally understand what you are going through. When I started at my facility no one even mentioned to me that charting is something that had to be done. It was kind of an optional thing. I truly can say it is a very important thing to do for many reasons. I simply went into excel and typed in all my residents names down the left side and put the activities for the day across the top. I keep the blank template on my computer and just type the activities for the day across the top and print it off daily. Charting is a pain to matter what. I have almost 90 residents so I understand what a pain it is, but it is so worth it.

I am an activity Director in Fl. To keep attendance records I photocopy the activity calendar... I work in a 120 bed facility so I make 120 copies. Then I write the residents name at the bottom and at the end of every day I go through and just highlight the activities they came to or write in that I did a 1:1 visit. I keep all of this in one 3 ring binder.

When it comes time to write my progress note I just turn to the calendar with that person's name and I can see what they have been coming to and on what day. It also helps me establish their patterns. What time of day they are up. What activities they prefer, etc. When the month is over I do the same thing for the next month. I refer back to these for 3 months when writing my notes.

In the past, I too just ran off the calendar and highlighted the activities each person attended. On the back a created a form I copied for any notes about the participant and or the activity. It worked well.

A few weeks ago I noticed this topic come up on this site and I read everyone's ideas carefully because I felt overwhelmed by my current process of participation charting.

I decided to adopt a member of this site's idea (sorry - can't remember who) to use the activity calendar with highlighting. She mentioned the highlighting system is great because it allows you to show all the times you encouraged residents to are doing your job.

Starting May 1st I'll be putting that charting system into effect!

Hi.......i am a new activity director at a SNF and i too think the forms we use are very old. i was wondering if any one had a res. participation sheet, room visit sheet, sensory stim sheet, or a monthly calender they would like to share with me for activity ideas. i know it is alot to ask for but i am new and would like to do the best i can to make the residents lives meaningful..... thanx Kim

I am a CTRS by trade, but have recently become an Activity Director at a CCRC, and would like any help I can find on Attendance forms. We only have about 26 Residents on our healthcare unit that we care plan, but it's hard to keep track in any uniform way the attendance of AL and Independent Residents. I need a simplified form if anyone has any suggestions. The only offer I got was a form that I've seen before that shows percentages by counting the amount of activities you have each month, and figuring it the levels of Residents, etc., and it still doesn't make any sense to me--HELP!!!
